Our offices require that all new clients fill out their health history and intake forms online. While we love providing this convenience for our clients, please plan on spending 20-25 minutes to fill out this form. Our form is a very detailed health history that allows us to address your health comprehensively. We make our sincerest effort to respect all of our clients’ time, so we ask that you fill out the online form before arriving to our office or else we may have to reschedule your new client evaluation. One last thing, this form is easiest to complete on a desktop/laptop computer and not a smartphone/tablet.
